How to Become a Patient

Inner City Health Center does not provide free care. Our uninsured pay on a sliding fee scale, based on the income and number of people residing in a household.

Registration Requirements:

·        A form of identification

·        Insurance card if insured by: Medicaid, Medicare, Colorado Access, CHP+, Access Advantage, and Sloans Lake

·        Proof of your address: Utility bill under applicant's name, if applicant has no bills please have a written letter signed by the title-holder stating you do in fact live on the property

·        Proof of income (if not insured): One month's worth of pay-check stubs from previous month's employment, and if not paid with check bring in a typed letter on the company’s letter head stating wages and pay dates

Registration times and hours are from Monday through Friday: 9am – 11am and 2pm-3pm
